Problem with VoIP and NAT

Hello, I'm trying to run a voip fone under a natted environment, to make 
the long short, it isn't working, I'm searching trough google, and I've 
found this link http://www.iptel.org/sipalg/, which basically says to use 
the patch-o-matic, and compily the sip-conntrack-nat patch, I have the 
kernel sources, and I've downloaded the correct iptables sourcer (1.3.8), 
and I'm using gentoo, but when I try to use the patch-o-matic, all 
patches fail, because of missing files...is there something I'm not 
seeing correctly?
Also, I already have the nf_nat_sip and nf_nat_conntrack modules loaded, 
but since that pages talks about the ip_nat_sip and ip_nat_conntrack I 
guess they are really diferent right?
Can someone please help me?
