Since the vast majority of pci cards only use one, using the other three tends to expose problems in marginal motherboards. (That's how I understand it, anyway. I'm no PCI expert, but I've seen similar problems with 4-port tulip, sundance, and e1000 cards). Are you using ACPI? I happen to have an older Via Epia board, and I've noticed that it disables one of the 4 interrupt lines completely in the bootup messages. I don't claim to understand what that means, but it sounds like bad news if you're trying to use all four.
