lshw sees 12GB RAM but system only using 8GB

[2.] Description

When I originally installed an additional 4GB RAM into my system a
year or two ago, the kernel recognized all 12GB just fine. Somewhere
along the line, however, the kernel stopped using all 12GB and started
using only 8GB (unfortunately, I have no idea when this occurred).

Specifically, "free" and cat /proc/meminfo only show 8GB of RAM, for
example:

as do other reporting tools (e.g., nmon).  Dmesg also shows only 8GB
in use by the kernel at boot:
