[PATCH 19/34] KVM: Drop superfluous __KVM_VCPU_MULTIPLE_ADDRESS_SPACE macro

Drop __KVM_VCPU_MULTIPLE_ADDRESS_SPACE and instead check the value of
KVM_ADDRESS_SPACE_NUM.

No functional change intended.
