[PATCH] mm: kfence: fix handling discontiguous page

The struct pages could be discontiguous when the kfence pool is allocated
via alloc_contig_pages() with CONFIG_SPARSEMEM and !CONFIG_SPARSEMEM_VMEMMAP.
So, the iteration should use nth_page().
