[PATCH v3 1/5] mm/huge_memory.c: remove dedicated macro HPAGE_CACHE_INDEX_MASK

Rewrite the pgoff checking logic to remove macro HPAGE_CACHE_INDEX_MASK
which is only used here to simplify the code.
