[PATCH 11/15] mm/hmm: rename HMM_PFN_DEVICE_UNADDRESSABLE to HMM_PFN_DEVICE_PRIVATE

Make naming consistent across code, DEVICE_PRIVATE is the name use
outside HMM code so use that one.
