There is also the case where the page is swapped in again but still allocated in the swap cache. So it's swap-backed, private and clean (because the copy in swap is still valid). But in that case it's not accounted to "Swap:" (presumably because Rss + Swap wouldn't add to the mappings size). I only care about consistency in /proc/smaps, but agree that an anonymous page that is not backed by swap-cache should always be dirty (in case it was cowed from the zero page at any point of course). Probably that inconsistency doesn't matter, as if it isn't swap-backed even a clean anonmous page can't be simply thrown away (in fact, "clean" or "dirty" doesn't have a meaningful semantics for anonymous memory IMHO).
