[RFC PATCH v3 14/26] ext4: factor out ext4_map_{create|query}_blocks()

Factor out two new helpers, ext4_map_query_blocks() query blocks
mapping, and ext4_map_create_blocks() allocate new blocks and map a new
extent, no logic changes.
