[RFC v4 0/7] Turn iomap_page_ops into iomap_folio_ops

Here's an updated version that changes iomap_folio_prepare() to return
an ERR_PTR() instead of NULL when the folio cannot be obtained as
suggested by Matthew Wilcox.
