The following functions are (now) unused:
 - __radix_tree_delete_node
 - radix_tree_for_each_contig
 - radix_tree_gang_lookup_slot
 - radix_tree_join
 - radix_tree_maybe_preload_order
 - radix_tree_split
 - radix_tree_split_preload
