[PATCH v2 3/3] ext4:Reimplement convert and split_unwritten.

v0->v1:
  -- ext4_ext_convert_initialized() zeroout whole extent when the extent's
     length is less than 14.

convert and split unwritten are reimplemented based on ext4_split_extent()
added in last patch.
