I have

    options cx88_alsa index=2

in my modules.conf to set my HVR3000 to be the 3rd sound card.  I also
have some similar alias statements to yours :

   alias snd-card-0 snd-intel8x0
   alias sound-slot-0 snd-intel8x0

   alias snd-card-1 snd-bt87x
   alias sound-slot-1 snd-bt87x

   alias snd-card-2 cx88_alsa
   alias sound-slot-2 cx88_alsa

But I think the first statement is important too.
