[PATCH 02/12] staging: comedi: ni_mio_cs: cleanup pcmcia_driver

For aesthetic reasons, reorder the pcmcia_driver variables and
add some whitespace.

Also, remove the unnecessary '&' before the function names. They
are already addresses.
