Same issue with ASUS X455LB (Qualcomm Atheros QCA9565 / AR9565). I believe this
is since 4.10.10, because previously, at the worst all I need to do (when the
bluetooth doesn't come back after resuming) is:

rfkill block bluetooth
<wait a few seconds>
rfkill unblock bluetooth

With 4.10.10 and newer, only the power cycle workaround works. Thank God I see
this report.
