We need the signature resolving key for each direction. And why did you include it as uint8_t[12]. They overall key is 16 octets. I think this should be bt_att_set_signing_key() and bt_att_resolving_key() and once they are set we can internally drop signed writes that do not fit. And we can also automatically sign if the signed write opcode is used. Do we need this distinction. I would like to be just able to register for a given opcode. And not all opcode. Just register for the one that you are interested in. This catch all type of things just end up with a big switch statement in the callback. I prefer having individual callbacks right from the beginning. This also has the advantage that if no specific callback is registered for an opcode, we can internally handle the response to reply with an error. So users don't need to provide all of them if they do not want to.
