[  143.010731] ==================================================================

This is because 'q_usage_counter' will not hold when bio_endio() is
called from error path, thus bio_endio() can concurrent with
blk_cleanup_queue():

t1                                   t2

bio_endio
 rq_qos_done_bio
  __rq_qos_done_bio(q->rq_qos, bio)
                                     blk_cleanup_queue
                                      rq_qos_exit
                                       ->rq_qos is freed
   -> uaf

Thus split the release of rq_qos to blk_release_queue() to fix the
problem.
