[PATCH V10 09/11] blk-mq: add blk_mq_hctx_handle_dead_cpu for handling cpu dead

Add helper of blk_mq_hctx_handle_dead_cpu for handling cpu dead,
and prepare for handling inactive hctx.

No functional change.
