I've consistently seen I/O timeout error in the kernel message and with fio output giving I/O error. Thanks a lot for looking into this, all the suggestions look good to me, meanwhile I've raised a question that ideal scenario should be in order to pass the testcase see [1]. As I'm getting device with 0 capacity at the end of testcase and there are still open questions.
