chained bio fixes and improvements

Hi Jens,

Brian Foster pointed out that chained bios might be clobbering an error
set in bi_error when a later chained bio completes successfully.  This
sounds unusual, but the way my new XFS writeback code uses bio chaining
it would actually be the mormal case, as we chain earlier to later bios
to make the code work.

While looking at the code I also found a couple additional improvements
that seemed worthwhile.
