MPU-401 ports are ISA devices; they are not listed in the PCI Plug&Play
information, and you cannot use a port address from there.

If your mainboard is recent enough, the MPU-401 is listed in the ACPI
Plug&Play information, and the snd-mpu401 module will automatically load
without any parameters.

If there is no ACPI information about the MPU-401, you have to manually
set the parameters, _and_ disable P&P ("pnp=0").  The port address and
interrupt can be set in the BIOS setup, and that is the only place where
you can look to determine their values.
