If you are using the alsa-backend of jack for audio, there is a commandline switch to also forward the alsa-hw midi ports to jack-midi directly in jackd. If not (because you use a firewire-device for example) a2jmidid has a commandline-option to also bridge the alsa-hw midi ports to jack-midi...
