Thanks! That was the problem.  Works perfectly now.

I built a version with jack. On Debian, this is the usual "apt-get source mplayer", then edit debian/rules to add the flags for jack, then debuild.

It's here if anyone wants a Debian amd64 build of mplayer with -ao jack:
