Its pretty easy, at the top of the inner loop the frequency is set, but it should be changed so that its updated every 4 or 8 samples, linearly interpolating between the previous value and the new one. It may needa bit of tweaking, eg. applying a lowpass filter to the linearly interpolated parameter, but I think just linear is fine. Its about 4 or 5 lines of code and hot hard, but tricky to explain.
