You would use a version of SLMODEMD-alsa-1.XX-YY.tar.gz or use
slmodem-source deb package which automagically creates everything for
you under ubuntu.

Marv can provide better instructions, or you may get them once you
update and run latest scanModem to generate the new ModemData.txt.
