This is a Conexant HSF modem.

I strongly recommend to correctly install the HSF driver from Linuxant which matches your Linux distribution and kernel.

Note that the driver on the Linuxant site is FREE but LIMITED (14.4 kauds, no FAX). You can later purchase a license which will unlock the limitations. I very strongly recommend to postpone purchasing the license until you have checked that the free version works. And no, unfortunately, there is no alternative to the Linuxant driver for HSF modems.
