The AgereSystems/LSI agrsm code supports compiling of a agrmodem +
agrsm driver pair.

Goto http://linmodems.technion.ac.il/packages/ltmodem/11c11040/
Thereat get the agrsm_howto.txt and  the agrsm-tools.deb packages.

The initial agrsm_howto.txt compiling steps are cogent to modems
with PCI ID:  11c1:0620

They use the agrsm-20090418.tar.gz package.
Unpack with:
$ tar zxf ag*.tar.gz
Within the new package do:
$ sudo make

The agrsm-tools sets a useful symbolic link and a agrsm-test utlity.
Install with:
$ sudo dpkg -1 ag*.deb
