scanModem recommends 11c11040 drivers:

For Debian/Ubuntu related systems, at
http://linmodems.technion.ac.il/packages/ltmodem/
there is an agrsm-tools_0.0.1_all.deb package providies for creation
of symbolic links
and usage information for modems using agrmodem.ko + agrserial.ko drivers.

Download the following debs which should work in your system:

NOTE:  There are two one with 14a_ and one with 24 only.  Don't know
what the difference between them is ?
This makes things a bit confusing since one does not know which one of
the two is better?  I hope Marv lets you know which one to download
and use, but in the meantime if you can download both.  Better be safe
than sorry.

If after installing the driver, the modem does not respond, please do
as scanModem suggests:

 The PCI slot 00:1b.0 of the modem card may be disabled early in
 a bootup process,  but then enabled later. If modem drivers load
 but the  modem is not responsive, read DOCs/Bootup.txt about possible fixes.
 Send dmesg.txt along with ModemData.txt to discuss@xxxxxxxxxxxxx
 if help is needed.
