With a CPU=x86_64 supporting installation,
there is no support for
     Primary device ID:  8086:293e
   Subsystem PCI_id  1025:0176
   Softmodem codec or chipset from diagnostics: 0x11c11040

because 64 bit code cannot be joined with a 32 bit component in the
software package.
Keeping your current installation, you would need a modem with a
slightly more expensive Controller Chipset, either external or
internal. Read InfoGeneral.txt about such modems

But if you should choose to completely replace your x86_64
installation, with a 32 bit Ubuntu installation,
the  0x11c11040 chipset modem can be supported

The currently used agrsm code is from updates at
http://linux.zsolttech.com/linmodem/agrsm/
the 	dkms-agrsm_2.1.80-5_i386.deb to
see an example at:
