Amin,

Your modem appears to be an ESS modem,

Predictive diagnostics for card in bus 00:0d.0:
	Modem chipset  detected on
NAME="Communication controller: ESS Technology ES2898 Modem "
CLASS=0780
PCIDEV=125d:2898
SUBSYS=148d:1030
IRQ=11
IDENT=ESS.com

 For candidate modem in:  00:0d.0
   0780 Communication controller: ESS Technology ES2898 Modem
      Primary device ID:  125d:2898
 Support type needed or chipset:	ESS.com

I am not sure if your modem is supported or not through Linux

http://linmodems.technion.ac.il/packages/ess/

I hope that a package from there will make your modem work, but I am
not certain that such a driver exists.

Regards,

Antonio
