 The modem codec file for the HDA card is: /proc/asound/card0/codec#1

At this time the only option is to install a 32bit  kernel+modules
version 2.6.24-19-generic to alternatively boot with.  Then from
http://linmodems.technion.ac.il/packages/ltmodem/11c11040/

you could use the agrsm-ubuntu8.04.1-2.6.24-19-generic.tar.gz
