If you can alternatively boot with Microsoft, do the modem diagnostics
as described in SoftModem.txt, and try to get the chipset information
and Driver Details.

The best guess at the moment is that your modem chip is on hosted on
the audio card, and is a Conexant type. The support software for Linux
is available at Linuxant.

So try the below and please report back.

 From  http://www.linuxant.com/drivers/hsf/full/downloads-ubuntu-x86.php
 download hsfmodem-7.68.00.07full_k2.6.24_19_generic_ubuntu_i386.deb.zip
 Under Linux unpack with:
 $ unzip hsfmodem*.zip
 Then install with:
 $ sudo dpkg -i hsfmodem*.deb
 Subsequently, the modem should be found with
 $ sudo wvdialconf /etc/wvdial.conf
 Edit in your personal information with:
 $ sudo gedit /etc/wvdial.conf
 and try dialing out with:
 $ sudo wvdial.
 See DOCs/Testing.txt  for details.
