Jacks' modem is an HCF as you write yourself !
Your instuctions are for HSF , not HCF !!!
In addition, are you sure that the Dell OEM HSF modem driver which you
quote works under kernel? I am not. If you are 100% sure, please let me
know.

The Dell driver quoted by Antonio does not work on your modem AS FAR AS
I KNOW.
You have no other choice than what I sent you before.
The driver downloaded from Linuxant is limited to speed 14,400 bauds, no
FAX.
To lift the limitations, you have to pay Linuxant 20$.
The fee is lifted for Dell users whose hardware and software match the
driver quoted by Antonio,
AS FAR AS I KNOW, again, this is unfortunately not your case.

For owners of a Dell PCs with Conexant HSF modems, a driver source package with full speed enabled is available, but requires driver compiling. Read DOCs/Conexant.txt

Under Linux unpack with:
$ unzip *.zip
Then install with:
$ sudo dpkg -i *.deb
Subsequently, the modem should be found with
$ sudo wvdialconf /etc/wvdial.conf
Edit in your personal information with:
$ sudo gedit /etc/wvdial.conf
and try dialing out with:
$ sudo wvdial.
