For reasons obscure, sometimes there is a
Waiting for Carrier
message despite the line
NO CARRIER

The
Cannot get information for serial port
is harmless.

Please read the wvdial.txt in the SLMODEMD package about other options to try.
Perhaps the first to test is adding an X3 ( don't wait for dialtone)
into the line Init2:
Init2 = ATQ0 V1 E1 S0=0 X3  &C1 &D2 +FCLASS=0
This compensates for low line voltages,

if that doesn't help, next try:
Init2 = ATQ0 V1 E1 S0=0 X3  &C1 &D2 =MS=34
which compensates for noisy lines
