There are two clear success reports usinglinux-image-2.6.21.0-22_2.6.21.0-22_i386.deb, which is indeed the onlycurrent choice for the 11c11040 chipset modems.  So your problem islikely that the x86_64 motherboard is not compatible with thekernel+drivers compiled for an i586 processor.
With respect to dialout under the Zoom V.92 PC, try shortening theLoginName to souzavonessen.
Also if you are not in the USA, you should set a Country Code line inwvdial.confInit2 = AT+CGI=hexadecimal_country_code
