The new scanModem data confirms the guess below,
Yours is most likely a Conexant modem.  You could install the rpm package for
 kernel-devel-2.6.22.5-31-default
to complete compiling support and use from
http://www.linuxant.com/drivers/hsf/full/downloads.php
the  	hsfmodem-7.68.00.10full-1.i386.rpm.zip
OR
upgrade to a kernel 2.6.23.14-64.fc7

Read the Conexant.txt and report back presently.
If a Conexant chip is confirmed, your data will be added to the
internal scanModem archive.
