Read the Smartlink.txt

Get the SLMODEMD.gcc.4.1.tar.gz  from
http://linmodems.technion.ac.il/packages/smartlink/

Unpack under Linux with:
$ tar zxf SL*.tar.gz
Move in with:
$ cd  SLMODEMD.gcc.4.1
read the documentation therein.  But your setup command is
$ sudo slmodemd -c USA  modem:1

The slmodemd acts through the already loaded  snd-intel8x0m  modem driver
