   Subsystem PCI_id  14f1:5051

The 14f1 is a  Vendor ID of Conexant,
implying almost certainly that the modem chipset is also Conexant
requiring the hsfmodem software.

More installation guidance will be output, if you fake a know Conexant
chipset with:
./scanModem  test 8086:284b 17aa:20ac
