Looks like the modem is supported via

http://linmodems.technion.ac.il/packages/smartlink/SLMODEMD.gcc4.1.tar.gz

download it, and read the instructions carefully, mainly

Get from http://phep2.technion.ac.il/linmodems/packages/smartlink/ the
SLMODEMD.gcc4.1.tar.gz and follow instructions therein.

Your modem setup command will be:
$  sudo slmodemd -c USA --alsa hw:0,6

output from scanModem was incomplete.  Report back if you need more directions.
