 14f1 is the Conexant Vendor ID, and 0x0x14f12bfa a softmodem chipset.

 Get a hsfmodem package through http://www.linuxant.com
provides an intial low speed driver set at:
http://www.linuxant.com/drivers/hsf/full/downloads-ubuntu-x86.php
 There is:
	hsfmodem_7.68.00.04full_k2.6.22_14_generic_ubuntu_i386.deb.zip
matching your kernel.

Unpack under Linux with:
$ unzip hsf*.zip
Begin the install with:
$ sudo dpkg -i  hsf*.deb
