12b9:1008       US Robotics 3cp803598                   serial driver supported

$   sudo wvdialconf  /etc/wvdial.conf
should detect the modem.
Afterward edit in your personal information with
$ sudo gedit  /etc/wvdial.conf
Then dialout with
$ sudo wvdial

 Read Testing.txt for details.
