Much thanks, as this is a new observation.
In fact modem:1 does work for the other AC'97 class modems.  So it
seems there is something exceptional about the MOT66 codec.

For the modems hosted on High Definition Audio cards, a specification like:
-------------------------------
00-06: Si3054 Modem : Si3054 Modem : playback 1 : capture 1
00-02: ALC883 Analog : ALC883 Analog : capture 2
00-00: ALC883 Analog : ALC883 Analog : playback 1 : capture 2
----------------------------
means that the setup line is:
# slmodemd -c USA --alsa  hw:0,6

Please retest your modems and report back.  If Ubuntu systems use:
$ sudo  slmodemd -c USA --alsa  modem:1,0
