Correct for cards in the family
 High Definition Audio Controller (HDA)
which can host a modem in Subsystems like the 103c:30bb

Your /proc/asound has the right files structure,
but all the files are empty. This is sometimes a copy problem, though
I don't understand it.  You audio appear to have life because

 Modem interrupt assignment and sharing:
 21:        247          0   IO-APIC-fasteoi   HDA Intel

But from failure of other diagnostics and slmodemd, I suspect your's
is a Conenant soft modem.  Run the modem diagnostics under Microsoft
to check.

For Gentoo, you will have to compile your own drivers.
Get help from the Gentoo List/info on compiling setup thereunder, if
necessary. Read the Conexant.txt.  An appropriate tarball will follow.
Test and report back presently.
