At http://linmodems.technion.ac.il/packages/ltmodem/kernel-2.6/Ubuntu/
get the ltmodem-2.6.12-9-386_8.26b1_i386.deb
and under Linux install with:
$ sudo dpkg -i lt*.deb

for your modem hardware
Class 0700: 115d:000c Serial controller: Xircom Mini-PCI V.90 56k Modem
     Primary PCI_id  115d:000c
 Support type needed or chipset: Agere.DSP

 The modem has a supported Lucent/Agere  Mars or Apollo DSP (digital signal
 processing) chipset. Support packages for 2.6.n kernels are at:
 http://phep2.technion.ac.il/linmodems/packages/ltmodem/kernel-2.6/martian/

 See AgereDSP.txt for Details.

When you upgrade to post 2.6.15 kernels, switch to the martian code
replacement for the  older ltmodem packages.
