If by chance your computer is made by Dell, you can get the full unlimited (56k, with Fax) Linuxant HSF driver for free from the Dell support Web site:

 10de:03f0 could be either a HSF or HCF Conexant modem, because of ambiguous
 use of the PCI ID by modem assemblers. First try the hcflinodem package.
 If the installation aborts, then use the hsfmodem package. Read

 The hsfmodem package serves a great variety of Conexant chipset modems.
