Jarno,

All COMM channels except lo (loopback) was be closed before trying to
use the modem.
determine ethernet address for proxy ARP"
shows that the modem is trying to use sevices for Ethernet and that
will not work.

Before trying a dialout display COMM channels with
$ ifconfig
Shutdown all except lo with commands like:
$ ifconfig eth0 down
or comparable action using the Control Panel,  Check again with
$ ifconfig

Finally do the dialout.  Below is an "ifconfig" showing an eth0 block
