Sorry, I had to leave my desk for a couple of hours.
There are at least 5 external versions of this modem:
RS232 which is not your case
USB ISDN: we cannot help you, we deal only with analog modems.
USB ADSL: the same
USB analog using a Conexant chipset.
USB analog using a SmartLink chipset.
For the last two we can help you.

Please try to use Windows as ModemData.txt had suggested and I replied.
You can also try to find *.INF files on your Windows describing the modem or the name of the driver file.

For SmartLink, download the file
Unpack with:                      tar zxf slmodem-2.9.11-20070204.tar.gz
Go into driver directory:         cd slmodem-2.9.11-20070204
Read instructions
Apply case of driver  slusb

You must first know which one to install.
If you have a little bit of Linux experience you can try one, remove it If failing, try the second, hopefully will work. You can do no damage doing that, but without experience you may well delete files not belonging to the modem. Thus I recommend making all possible effort to boot Windows and identify the modem as explained in ModemData.txt, and if you do not have Windows installed on the same machine, search your Windows drivers CD.

clicks on:
Start > Settings > Control Panel > Classical View (for Window XP) > System > Hardware > Device Manager > Modems > Click on the + > Modem. Double click to expand the graphic. Manufacturer information may be displayed. For example, CXT stands for Conexant. Click the Diagnostics Tab. Record any hardware ID or vendor and device information. Next do the Query Modem and record the ATI specifications displayed such as:
    ATI3 - Agere SoftModem Version 2.1.22
    ATI5 - 2.1.22, AMR Intel MB, AC97 ID:SIL REV:0x27
Try to identify the modem setup file, with name perhaps MODEM.INF.
If may contain chipset Vendor informaton.
