Michael,
Your question is ambiguous.
Do you want to automatically start the modem DRIVER slmodemd or to also DIAL to your ISP?

To only start slmodemd:
1- type which slmodemd to see its full path such as /usr/bin, possibly something else 2- in your directory /etc/rc.d/init.d (perhaps /etc/init.d) create a file named slmodemd containing these two lines

!#/bin/bash
/usr/bin/slmodemd --alsa -c GERMANY  hw:1 > /dev/null 2&>1 &

Save it, make it executable with command   chmod a+x slmodemd

Go into directory rc5.d
Type  ln -s /etc/rc.d/init.d/slmodemd S99slmodemd

Your slmodemd will restart at boot

If you insist on dialling automatically, find the path to wvdial with
which wvdial   (giving perhaps /usr/bin/wvdial)
and add one line:
/user/bin/wvdial > /dev/null 2>&1 &
to file /etc/rc.d/init.d/slmodemd

To use wvdial you MUST have priorily created /etc/wvdial.conf by using
wvdial WHILE slmodemd IS RUNNING, and edited the file /etc/wvdial.conf

Of course you may already have done that if you already use wvdial.

Lacking a dsp (digital signal processing) chip, the modem is a software intensive or "softmodem" type. Its primary controller manages the traffic with the CPU. But the software needed is specified in the Subsystem.

 An ALSA (Advanced Linux Sound Architecture) modem driver:  snd-intel8x0m
 provides Low Level support enabling contact with the modem hardware.
 For all BUT Conexant chip soft modems (using hsfmodem software)

Download from http://linmodems.technion.ac.il/packages/smartlink/ the package SLMODEMD.gcc4.tar.gz having a compiled slmodemd. Unpack under Linux with:

 and read instructions therein. But briefly, the modem is setup with command:
slmodemd -c YOUR_COUNTRY --alsa modem:1 reporting dynamic creation of ports:
