There is a login protocol problem, rather than a driver problem.Looking at the end of /var/log/messages following a failed attempt maybe information.
The error code numbers that may therein be mentioned, refer to thoseexplained towards the end of$ man pppd
Some IPs  require a LoginName format like  VR\LoginNamewhere VR is for example VERIZON.net
Look at the Postintall HowTo at http://linmodems.technion.ac.ilJacques may have suggestions, he is more knowledgeable on protocol issues.
Also make a Port/Modem log of a dialout under Microsoft, to see howthat negotiation is managed.
