The
SLMODEMD_DEVICE=slamr0
should be
SLMODEMD_DEVICE=hw:0,6

I mean the slmodemd loading not snd-hda-intel.
How can I insert this on bootup progress :

I've tried edit the /etc/sysconfig/slmodemd into this :

On your Mandiva system, your snd-hda-intel probably autoloads during bootup.
Check with:
$ lsmod | grep snd-hda-intel
But you could add snd-hda-intel to the list in /etc/modules
